About the Advanced Practice Provider Role Upperline Health is seeking a Nurse Practitioner or Physician Assistant to assess and treat patients with complex wounds, or post-surgical conditions in our Birmingham clinic. Our Advanced Practice Provider will work within the scope of their license to care for patients as well as providing management of additional conditions as needed. Wound care providers specialize in assessing and treating patients with complex wounds. They will work closely with members of patient care teams and collaborating physicians to ensure that personalized treatment plans are followed correctly. Common types of patient wounds include ulcers, burns, pressure injuries, diabetic foot wounds and skin tears. This will include a range of clinical tasks such as performing a physical examination, assessing the condition of the wound, debridement, prescribing medications and DME, utilizing laboratory tests and ultimately being responsible for optimal healing. Ideal candidates will have had prior experience working in a clinic or hospital setting and have experience with wound care. The candidate must demonstrate compassion for patient needs, have great attention to detail and thrive in a collaborative environment. What Youll Do Provide Upperlines services in a clinic setting with the possibility for home visits as well Conduct a thorough initial assessment of patient, review his/her medical history, listen to the patients concerns, perform a physical examination, order medical labs as needed, develop and implement a clinical plan of care Provide procedures such as debridement and cleaning of wound, bandage wounds, provide appropriate offloading as well as coaching patients on best practices to improve their healing Work collaboratively with the larger patient care team to have a comprehensive care plan including ordering services such as HH, DME, and biologics Provide advice and instructions on preventive care and you will also educate patients and families about general wellness care, medication usage, side effects, diet and nutrition, medical adherence and educate patients and caretakers on wound care, infection and injury prevention, wound dressing changes, and pressure ulcer (bed sore) care Maintain patient medical records and medical documentation consistent with state regulations and Upperline Health standards and policy Participate in continuing education as required by the state Board of Nursing Prescribe medication as permitted by the state Board of Nursing Other related duties as deemed necessary Qualifications of the Advanced Practice Provider Family or Adult Nurse Practitioner or Physician Assistant with appropriate certification and licensure is required Previous experience in wound care, trauma, orthopedics or internal medicine Wound care certification or experience is preferred Possession of DEA registration or eligibility required CPR certification preferred Upperlines APPs must have strong compassion for our patients and their families Thrives in a fast-paced, high-stress, demanding medical environment Excellent oral and written communication skills to ensure that each of your interactions with patients, family caregivers, clinicians, and team members is effective and meaningful.
