encompass: Assessing and evaluating patients to determine their
eligibility for care. Employing available tools and resources to
develop customized care plans that outline the necessary level and
frequency of treatment. Effectively communicating with the care
team, physicians, payors, patients, and families to address care
requirements and implement the patient’s care plan. Providing
high-quality skilled care while overseeing the contributions of
other team members. Maintaining daily documentation of care
delivery in our EMR system to ensure precise records. Fostering a
