The Care Manager plays a vital role within our interdisciplinary
team, responsible for assessing, planning, advocating, and
coordinating patient care from the moment of admission through to
discharge. This position ensures a smooth transition after
hospitalization, aligning with our organization's mission, values,
and service standards. Our Care Management model emphasizes
effective transition planning and oversight of length of stay,
focusing on enhancing patient experience, safety, and quality of
care through the use of performance metrics and best practices.
Position Overview: Required Education: • Graduate from an
accredited Nursing School • Bachelor of Science in Nursing
Experience: • Preferred 2-3 years in an acute care hospital
environment • Experience in Care Management or Utilization
Management is a plus Licensure: • Must hold a current nursing
license in the state of employment Certifications: • Basic Life
Support (BLS) certification required at the time of hire or within
90 days of employment • Certification in Case Management is
