Job Description Job Description Position Summary: The Stepping
Up Case Manager provides support to individuals with serious mental
illness (SMI), substance use disorders (SUD), or co-occurring
conditions as they transition from incarceration back into the
community. This position is part of the Stepping Up Initiative,
aimed at reducing the number of individuals with behavioral health
conditions in jails. The Case Manager will work with incarcerated
individuals nearing the end of their sentence to provide reentry
planning, support, and linkage to essential community services that
promote recovery and long-term stability. Key Responsibilities:
Collaborate with correctional facility staff to identify and engage
individuals with SMI/SUD nearing release. Conduct comprehensive
assessments and develop individualized reentry plans addressing
treatment, housing, employment, and other key needs. Provide case
management services including coordination of care, advocacy, and
connection to behavioral health and social service providers.
Assist clients in accessing benefits, securing identification, and
preparing for community reentry. Coordinate with probation/parole
officers, community treatment teams, housing providers, and peer
support specialists. Monitor client progress post-release,
adjusting service plans and supports as needed to reduce recidivism
and promote recovery. Maintain timely, accurate, and confidential
documentation in accordance with agency and regulatory standards.
Participate in Stepping Up-related meetings, cross-system
collaboration efforts, and stakeholder communication. Promote a
trauma-informed, recovery-oriented, and person-centered approach in
all interactions. Respond to and manage behavioral health crisis
calls as assigned, providing immediate support, triage, and linkage
to emergency or crisis services when appropriate. All other duties
as assigned Qualifications: Bachelor’s degree in Psychology, Social
Work, Human Services, or a related behavioral health field.
Experience in crisis intervention, behavioral health, or social
services is strongly preferred. Strong communication and
interpersonal skills, with the ability to remain calm under
pressure. Understanding of trauma-informed care, mental health
systems, and community resources. Valid driver’s license and
reliable transportation required. Availability for flexible
scheduling, including nights, weekends, and on-call shifts. Ability
to work both independently and collaboratively as part of a
multidisciplinary team. Work Environment & Schedule: This is a
non-exempt, full-time position. This position is community-based
and includes travel to locations throughout the service area in
response to behavioral health emergencies. Standard hours are
Monday through Friday, allowing for flexible scheduling to include
nights, weekends and on-call shifts. This position requires
in-person work and is not eligible for remote or hybrid work
arrangements. Transportation: Provide transportation for consumers
as needed using agency or personal vehicles. Maintain a safe
driving record and valid driver’s license with personal liability
insurance. Follow all agency transportation policies, including
seatbelt usage, safe cellphone practices, and vehicle
documentation. Immediately report any vehicle issues, accidents, or
violations to appropriate personnel. In-Service Training &
Continued Education Requirements: Participate in all required
agency in-service and ADMH trainings Commitment to ongoing
professional development to enhance skills and stay informed on
best practices in behavioral health. Completion of onboarding and
any continuing education requirements as directed by the agency or
supervisor. Ongoing professional development to maintain best
practices and compliance with evolving standards of care.
