Community Outreach/Population Health Manager

We Are Hiring:
Location: Ascension St.Vincent's Population
Work Schedule: Monday-Friday Day Shift
What You Will Do:
Plan, oversee, and direct staff for
outreach and community development programs.
What You Will Need:
- Build collaborative working relationships among community
agencies/partners. Plan, evaluate and identify needs for
neighborhood outreach services.
- Manage and coach staff in the development and implementation of
departmental policies, procedures, and programs. Approve hires,
terminations, promotions, demotions, pay, and associate status
- Monitor risk management concerns of program operations and
consult with legal counsel on maintaining compliance with local,
state and federal regulations.
- Design, monitor, and manage the department budgets.
- High school diploma/GED with 2 years of experience, or
Associate's degree, or Bachelor's degree required.
- 3 years of experience required.
- 5 years of experience preferred.
- 1 year of leadership or management experience preferred.
-Primary responsibility of this role is to lead a team of
navigators who provide phone based population
health management (post acute & proactive) to a number of
- Care management experience is strongly preferred.
- Must have strong computer skills with examples of leveraging
technology to improve patient outcomes.
